Those of you who attend LAUSD schools may find
that your coach will not be able to provide you
with equipment for the game.
Other schools may have their equipment out
for recertification.
If you run into either of these problems,
not to worry!
We will be able to supply you with what you need.
Just make sure you get this form back to us in
time for us to get your stuff for the
first practice.
